The plan covers drugs that require a written prescription. For each eligible prescription fill, you pay a co-pay which is $5.00 plus the dispensing fee.
Reimbursement for reasonable and customary costs of prescribed lenses or contact lenses every 4 years (every 2 years for children under 21), or when there is a reasonable prescription change. $150 for frames every 4 years (every 2 years for children under 21).
$1,800 annual max for all practitioners combined, ($10.00 co-pay applies to certain services).

Life Insurance equals to two times your annual earnings until age 70. Under this program your spouse is insured for $10,000 and each dependent child is insured for $5000
If you are unable to work due to illness or injury our Long Term Disability provides a benefit equal to 70% of your pre-disability monthly earnings.
Optional Life Insurance for You and/or Your Spouse. Coverage is available in units of $10,000 to a maximum of $500,000.00. We also offer life insurance for dependent children.
The coverage provided a benefit to you in the event of accidental injury or to your beneficiary in the event of your accidental death. Coverage is available in units of $10,000 to a maximum of $300,000
This coverage provides a lump sum payment in the event that you are diagnosed with a critical illness. Coverage is available for you and/or your spouse in units of $5000 with a minimum of $10,000 and a maximum of $150,000
